At different times in our lives, we may wish to reach out to others to offer support, guidance and compassion when they need it most. Consider Cedar Home Health, Hospice and Pathfinder team members to be your special partners, holding out their hands to you in service and hope. Cedar's five programs are each designed to support you in unique ways along the journey:
Cedar Home Health care is your partner for short-term nursing and therapy in your home or assisted living residence to help with recovery after a hospitalization or managing a chronic illness.
Cedar Palliative Care is a unique program that provides comfort care and support to those seeking treatment for a life-limiting illness.
Cedar Hospice provides dignity, comfort and compassion to patients and their families faced with a terminal illness.
Cedar Pathfinders, our Certified Geriatric Care Managers, are advocates and partners for aging seniors. They are trained and experienced in helping people navigate the complex landscape of health care services and resources, answer Medicare questions, or perhaps find the right living situation for someone based on their individual needs.
Cedar Supportive Care serves our clients with Personal Certified Nurse Aides and Homemaker/Companion partners who help with everyday tasks — even if it's for as little as one hour per day, a few days a week.
Cedar Home Health, Hospice and Pathfinders is a division of Cedar Community, a not-for-profit, Christian organization serving others since 1953.
Our mission: To glorify the Lord while providing spiritual, physical and emotional comfort to the individual and their family during illness or end-of-life care.
Cedar serves anyone 18 and older residing in Washington, Ozaukee, Waukesha, Fond du Lac, Dodge and Sheboygan counties.
